Corrective Action(s): Consult with Pest Technician
Install more traps in front area under ice machine, inside sushi area cupboard, etc.
Install fruit fly traps in bar and waitress stations throughout as some have dried up and some are missing from these high activity areas.
Chemically treat the drains
Have the entire facility treated for Cockroaches - MULTIPLE TIMES. Consult with pest technician as to number of treatments recommended by Pest technician. This is an ongoing infestation that needs to be dealt with once and for all.

Observation: Staff are continuing to leave dirty dishes, food debris and whole food items out overnight instead of cleaning facility prior to leaving for the night.
Food debris and splatter found on walls and sides of equipment throughout.
Staff are not cleaning wok and surrounding grill station prior to leaving for the night. Staff are not emptying grill trough prior to leaving for the night.
Crab leg and dirty pot left out overnight on burner
Crab leg found under hot water tank. Mice are taking food from one point in the restaurant to another!!!

Grease trap has not been cleaned since September of 2016 - HAVE GREASE TRAP SERVICED IMMEDIATELY!!

Corrective Action(s): DO NOT REPEAT THIS VIOLATION.
Leaving food debris, dirty dishes, grease, uncleaned food, leftover food out overnight is contributing to the ongoing pest infestation.
This practice is unacceptable.

Observation: Clean all rodent droppings and dead cockroaches found in traps IMMEDIATELY.
Use 200ppm Chlorine solution.
Grills in exhaust canopy need to be cleaned DAILY
Fronts of deep frying units are COVERED in grease - routinely clean this area

- Freezers achieving temperatures of less than or equal to -18 degrees Celsius
- Hot Holding achieving temperatures of greater than or equal to 60 degrees Celsius
- Dishwasher achieving final sanitizing rinse of greater than or equal to 72 Degrees Celsius (78.3 degrees Celsius observed) at the dish
- Sanitizer solution in use throughout food preparation areas and at concentrations of greater than or equal to 200ppm Chlorine
- Proper thawing techniques observed
- Ice machine and scoop in good sanitary condition
- Proper re-heating methods observed for previously cooked food items
- Raw meat stored away from Ready to Eat food items
- Pest Technician records available on site

NOTE: facility needs to be deep cleaned on a weekly schedule. Currently hard to clean areas such as deep frying units, sides of equipment, exhaust grills, etc. are ignored. Significant improvements are still required at facility. Current pest infestation is a result of staff's lack of nightly cleaning. UNACCEPTABLE.
