211 - Semi critical items not adequately cleaned and disinfected
Observation: .
1) Magnetic water circulation fans for foot baths have debris and grit inside and one has mould on the inside front cover around the threads used to secure the two pieces of the fan together.
2) Bleach bath for nippers, clippers etc is not deep enough to allow full immersion of equipment.
Corrective Action(s): .
1) ALL parts of the foot baths must be cleaned and disinfected between EACH client, including the inside of the magnetic fan assembly.
2) Bleach solution must be deep enough to allow full immersion of ALL equipment for the entire time required

Observation: Stainless steel instrument for blackhead removal and q-tips in open container in treatment room
Corrective Action(s): All items must be protected from contamination at all times, make sure all multi-use (i.e. blackhead removal instrument) are disinfected between uses and that both this and all other (i.e. q-tips) are kept in containers with tight fitting lids to prevent both microbial and physical contamination.

Observation: No hand towels available at the hand sink in the treatment room.
Corrective Action(s): SINGLE-USE hand towels, whether cloth or paper, must be provided for hand drying.

330 - Products / instruments not protected from contamination
Observation: Several (>4) wax strips in garbage container - only one applicator.
Corrective Action(s): All contaminated wax pots to be discarded and operator to ensure that ALL staff do not re-use applicator strips - one strip of cloth; one applicator.
DO NOT put used applicator back into warm wax UNLESS, the wax pot being used is for that client only and discarded when the service is completed.

NOTE: This infraction has been an ongoing, progressive enforcement implemented. Written order issued.
