210 - Food not thawed in an acceptable manner [s. 14(2)]
Observation (CORRECTED DURING INSPECTION): Two frozen bags of meatballs observed thawing at room temperature on top of chest freezer in by the dishwashing area. Multiple containers of soup observed thawing at room temperature in a tray next to the preparation area. All items are measured at or below 4'C during inspection. Staff placed all items in the cooler at 4'C or lower during inspection.
Corrective Action(s): Do not thaw potentially hazardous foods in the danger zone (4'C to 60'C). Thaw potentially hazardous foods using one of the following methods to prevent time-temperature abuse, bacterial growth and/or toxin production:
- under cold running water in the original packaging
- under refrigeration the night before (at 4'C or lower)

304 - Premises not free of pests [s. 26(a)]
Observation: Multiple house flies observed on site during inspection. Two sticky yellow fly traps are observed covered with house flies and observed located close to food preparation areas. Staff stated that a pest control company is contracted, however reports were not verified during inspection.
Corrective Action(s): Remove current house fly traps in place. Contact pest control company to assess and place more traps in non-food preparation areas (recommend UV fly trap close to the back door). Replace sticky yellow fly traps more frequently to prevent potential physical contamination of food. Keep all food covered at all times. Keep all doors and windows closed as much as possible to prevent fly entry on site. Submit pest control reports from the last two months to the health inspector for review.

Observation: The following areas are observed in unsanitary condition with food debris and/or grease build-up:

1. All wipeable vinyl backsplashes behind hand washing stations
2. Wooden floors in dry storage room observed with sticky debris
3. All cooler and freezer door handles
Corrective Action(s): Clean and sanitize the aforementioned areas to remove food debris and or grease build up. Keep a record of cleaning and sanitizing the aformentioned areas. Ensure the premise is maintained in a sanitary condition to prevent potential contamination of food.

Observation: 1. Ceiling light in food preparation area is observed missing a light sheild/plastic protector.

2. Floor tile upon entry into kitchen from the seating area is observed broken.
Corrective Action(s): 1. Repair the light cover in the food preparation area to ensure the light fixture is protected by a plastic sheild/cover to prevent potential physical contamination of food.

2. Replace the broken tile in the kitchen to ensure the floors are easy to clean and sanitize (as per approved floor plans).

Observation: In operator's absence, no staff on duty has FOODSAFE Level 1 or equivalent training. Staff stated that one food handler at the back will take the course in the next few weeks.
Corrective Action(s): One staff member on site must apply for FOODSAFE Level 1 training or equivalent. Send a copy of the invoice/receipt which confirms a staff member on site has applied to the health inspector.

- Hand washing stations are supplied with hot and cold running water, liquid soap and paper towel
- Chemical dishwasher in kitchen measured at 100 ppm chlorine concentration with test strip
- Chemical glass washer at bar measured at 50 ppm chlorine concentration with test strip
- 2-compartment sink available with drain plugs
- QUATs sanitizer in spray bottles (X6) measured at 200 ppm QUATs concentration with test strip
- All food observed covered and stored at least 6 inches off the floor
- First-in-first-out method is implemented (time stamps available on food containers)
- Equipment stored in a sanitary manner
- Chemicals are stored away from food
- Food handlers observed wearing non-latex gloves
- Pest control company is contracted
- Health permit posted in conspicuous location at bar
