Corrective Action(s): A PREempt CS20 solution with an expiry date is required for disinfecting the loop extractor. Obtain a new bottle of PREempt CS20 solution immediately. Reminder, this solution does not require dilution and loop extractor to be soaked for 20 minutes. Routine inspection: Threading: All threads single use. Waxing: All wax pots and trolleys maintained in a sanitary manner. All sticks and strips single use. Treatment room bed paper liner single use. Roller type wax applicators are single use. Eyebrow tinting: Services done by trained individual. Refectocil products have adequate warning labels. Hair: o-phenol clippercide available for use. King Research barbicide available for use. Both hair wash stations equipped with backflow prevention devices. Hair removal by IPL: Warning sign posted on door. Laser machine equipped with an emergency on/off button. Key required for operation. Hand pieces disinfected with 70% isopropyl alcohol. Client goggles completely opaque. Mirror is covered with a linen sheet. Razors single use. Facials: Tissue and fingers used to remove blackheads. Observed a loop extractor in service tray (see above violation). Microdermabrasion and hydrafacial services not offered at this time - staff currently in training. Reminder: diamond head microdermabrasion pieces must be disinfected in PREempt CS20 solution after use. *** Provide an updated infection control plan for these new services.*** Other: Utility sink available for use in the back area. Laundry services done onsite. Hand sink available for use. Liquid soap and paper towels provided.
