Observation (CORRECTED DURING INSPECTION): - Sanitizer concentrate dispenser empty. 0 ppm QUATS detected in all sanitizer buckets. Date and Time Stamp labels for sanitizer buckets not used per Sanitation Plan. Employees did not verify sanitizer concentration.
Corrective Action(s): - Replace sanitizer concentrate at dispenser.
- Refill all sanitizer buckets.
- Use QUATS test strips to verify sanitizer concentration (minimum 200 ppm).
- Use Date and Time Stamp labels for sanitizer buckets per Sanitation Plan.

Observation: - Thorough cleaning required in the following areas:
- Moisture and dirt accumulation under and behind 3-compartment sink and dishwasher area.
- Food debris and grime accumulation under and behind baking/ donut glazing area, ice machine, stove burners and all storage shelving.
- Food debris and grime accumulation under counters and on floors in front service area.
Corrective Action(s): - Thoroughly clean the above noted areas and all other hard to reach areas.
- Regularly maintain a higher level of sanitation throughout the entire premises.

**Most recent pest control service report (dated 03-Jan-2024) reviewed. Pest control service technician reported "poor sanitation" and made the following recommendations:
- Thorough sanitation and sweep out of all the appliances underneath shelving and racks.
- Conduct deep sanitation and sweeps underneath all equipment and shelving to eliminate food competition for pests in the area.

- Food storage practices appear satisfactory. Food is protected from contamination.
- Interior of ice machine and ice storage bin appears maintained in a sanitary condition. Ice scoops are stored in a sanitary manner.
- Drink mixing spoons are stored in ice water between uses. **Ensure that Date and Time Stamp labels are used per Standard Operating Procedures.**
- High Temperature Sanitizing Dishwasher: 72.5C at dish surface
- General sanitation in the dining room area appears satisfactory.
- Monthly pest control services provided by a pest control management company. Most recent report (dated 03-Jan-2024) reviewed. **Follow all recommendations provided by your pest control service technician.**
- FOODSAFE requirements are in compliance. Shift Supervisor has valid equivalent to FOODSAFE Level 1.

**Health Operating Permit Replacement Form provided at time of inspection. Valid Permit decal must be affix on Permit to Operate. (decal is currently affixed on City of Burnaby Business License).** Submit completed replacement form along with payment to Fraser Health to receive a replacement Permit to Operate.
