Observation:
- Ice machine - interior observed with biofilm.
Corrective Action(s):
- Turn the ice machine off, empty all ice out. Clean and sanitize the interior surface of the ice machine. Pink residue/film must be removed.

Observation:
- Staff nor operator able to demonstrate proper warewashing method involving sanitizer step. No drain plugs available.

- Equip well fitted drain plugs for 2 compartment warewashing sink. Staff should be able to demonstrate proper warewashing method.

Observation:
- One domestic fridge in the way to the back kitchen. This blocks the path into the back kitchen.
Corrective Action(s):
- Ensure to move the fridge OR place adequately sized commercial cooler/freezer unit to allow minimum clearance area. Dead space behind the fridge is hard to monitor or clean.

Observation:
- Observed dust build up on portable fan cover while the fan was blowing air into few containers of chopped watermelon.

Observation:
- Cockroaches found on glue boards.
- Live cockroaches observed behind bananas by the front display.

- Pull out all items from shelves, cabinets, counters. Clean and sanitize all surfaces.
- Hire a pest control company to provide inspection and treatment services. Provide a copy of the service report to the undersigned.

Observation:
- Excessive amount of food debris and dust observed on the floors underneath the equipment, on shelving compartments of fridges, surfaces of cooler doors, inside batter containers.
- Sticky residues observed around & inside the oven.
- Clutter observed throughout the premises.
- Number of unused equipment observed in the kitchen and the front area.
- Boxes of food products placed directly on floor.
- Cleaning equipment observed next to stove.
Corrective Action(s):
- Clean all the areas noted above.
- Organize the items out in the open. Place them in adequate compartments.
- Remove any unused equipment from the premises.

Comments

- All handsinks are fully equipped. Both soap dispensers at the back handsink is not working well. Ensure to unblock the dispensers OR replace the soap dispensers.
- All coolers at 4C or below.
- Freezer units at -9C to -14C.
- Sanitizer in bucket measured 200ppm chlorine.

*Equip drying racks warewashing sinks.
*Deep cleaning required.

Closure order issued due to staff incompetency, lack of sanitation and pest issues.
Contact the undersigned for re-opening once all corrections are made.
