Observation: Coolers lids stored on the dirty floor and mice box trap at the time of inspection.
Corrective Action(s): Immediately. All lids must be cleaned and sanitized; and stored in a sanitary manner/off the floor.

Observation: Mice droppings found in the corner with the utility box and storage shelves; near the high temperature dishwasher; and around small domestic cooler/sushi prep. line.
Old/dry rodent found in the box trap near the small domestic cooler.
Corrective Action(s): Today. Clean all mice droppings and sanitize floor, equipment, and walls with Bleach solution.
Notify a pest provider about the issue. Invoices/rodent treatment/ must be presented at the time of a follow up inspection.

Observation: Food debris and grease behind the hot cooking line and around the small domestic cooler/sushi prep. line.

Observation: Ripped plastic cover with food debris in cracks on the chest freezer with a damaged gray duck tape on the top.
A storage shelf above the chest freezer is damaged by water.
Corrective Action(s): The damaged surface must be replaced by a food grade durable material, which easy to clean and sanitize.
The storage shelf has to be replaced.
