Observation: Dust build up noticed on the ventilation system ducts, white wall as well as on the fan cover in walk-in cooler.
Corrective Action(s): Thoroughly clean the ducts and surrounding walls to remove dust and debris. Fan unit and fan cover must be cleaned weekly. (By Dec. 15, 2016)

* All food items, utensils, disposable cutlery and boxes etc. must be stored on shelves at least 6 inches off the floor. Absolutely no food or food related
items to be stored on the floor.
* No temperature and sanitizer logs maintained since June 2016. Ensure to monitor and record cooler temperatures, hot food temp and dishwasher sanitizer levels
at least once a day.
* Replace worn out foam around the doors in the sliding door cooler (By Dec. 25, 2016)
