Observation (CORRECTED DURING INSPECTION): 0 ppm chlorine sanitizer was measured in the labelled sanitizer pail in the sushi bar. Operator informed they had setup the chlorine sanitizer at 8 AM today and had not changed it.
Corrective Action(s): Operator setup 100 ppm chlorine sanitizer to sanitize preparation surfaces. Ensure to change the chlorine sanitizer and wiping cloth frequently throughout the day (e.g. every two to three hours and when cloudy or diluted) as needed in order to maintain it at 100 ppm chlorine sanitizer. Recommended: Obtain chlorine test strips to monitor the concentration of chlorine sanitizer.

Follow up inspection:
-Handsink in the sushi bar was accessible for handwashing purposes.
-Spray bottle containing detergent was labelled as "soap" to identify the contents.
-Sushi rice at ambient temperature in two containers had a pH below 4.2.
-Non-sushi rice was hot-held at or above 60 degrees C.
-Tempura was undergoing cooling inside the upright cooler.
-Potentially hazardous food was not being cooled at room temperature.
-100 ppm chlorine sanitizer was available in the sanitizer pail in the back food prep. area.
