Observation: Front swing gate - a hole has developed in bottom corner of gate allowing for pest entry.
Corrective Action(s): Modify gate to seal off hole to exclude pest entry.

Observation: Trace mouse droppings observed along back wall under dishwashing sinks, between undercounter cooler and sandwich display cooler, and in corners of back dry storage room.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
