Observation:
The following equipment is not maintained in a sanitary condition:
1. Entire cookline - under, behind and front of cookline.
2. Dishwasher propellers.
3. French fry slicer.
4. Inside all coolers.
5. Inside all freezers.
6. Under and behind all sinks.
7. Doors and handles of all coolers and freezers.
Corrective Action(s):
Clean and adequately sanitize all areas listed above. To be corrected by January 16, 2017.

Observation (CORRECTED DURING INSPECTION):
1. High temperature dishwasher not properly washing and sanitizing dishes.
2. No sanitizer available at the time of inspection.
Corrective Action(s):
1. Use the 3-compartment sink for manual dishwashing to adequately wash, rinse and sanitize (100-200 ppm bleach) dishes.
2. Prepare a 100-200 ppm bleach sanitizer solution.

Observation:
1. No soap and paper towels at hand washing station beside upright freezer.
2. No soap and paper towels in employee washroom.
Corrective Action(s):
Ensure all hand washing stations are fully equipped with liquid soap, paper towels and hot/cold running water at ALL TIMES. Liquid soap and paper towels need to be in dispensers. To be corrected immediately.

2. All light fixtures in kitchen.

Clean all areas listed above. To be corrected by January 16, 2017.

Cardboard found on multiple shelves in various coolers and dry storage areas.
Corrective Action(s):
Remove cardboard from all shelving. Cardboard is not an approved food contact surface. To be corrected today.

Observation:
After 5 attempts, high temperature dishwasher still not sanitizing with >71C at dish level.
Corrective Action(s):
Repair high temperature dishwasher so that dishes are sanitized with 82C or greater for 10 seconds at the manifold (>71C at dish level). To be corrected by January 16, 2017.
