Observation: Cockroaches observed in the following areas: above walkin cooler 3, under handsink 1, by back ventilation hood 2, along steel flashing above fuse box 1, behind fuse box 1, behind upright freezer 2 dead, trap behind ice machine several, under front beverage dispenser 2 dead and back prep counter 1.
Corrective Action(s): Continue to seal around all pipe - wall penetrations, seal crack in corner behind clean dishware storage.

Observation: Additional cleaning required in the following areas: surfaces above walk-in cooler, underside of back handsink, dishwasher dish rack, pop syrup rack, behind upright beverage cooler, behind front cooking line around metal supports, around door hinges of grill prep cooler, underside of grill side table, wires / conduits behind ice machine and around floor drain behind ice machine..
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
