Observation: Many mice droppings noted in facility (Mice infestation). Droppings noted on plates, behind and around plates, in almost every corner in this facility. Packet of poison underneath 2-door display cooler noted by backdoor.
Corrective Action(s): Operator must remove all droppings, sanitize areas, and monitor. Take everything out from shelving, clean area, and wash everything in facility (plates, towels, utensils, etc).
Do not use poison in facility.

Pest control company comes in once a month. Increase visit to once a week until mice problem is getting better. Must see new contract in follow-up inspection.

Observation: Back door by kitchen was not closed properly (with a visible gap), and screen door broken.
Corrective Action(s): This is one way mice can get into facility. Operator must try all methods to prevent mice from entering facility.

Observation: Missing tiles by dishwashing area where mop bucket is stored. Raw wood exposed.
Corrective Action(s): This infraction was noted in last routine inspection (March 2017). Operator was given until April 2017 to replace missing tiles. This is still not done. Failure to complay may result in further enforcement action.
