Observation: 1. Pooling water still observed by hot water tank.
2. Grease build-up still observed under grill in kitchen area.
Corrective Action(s): To prevent the attraction of pests:
1. Pooling water may be the result of a leak. Discover the source of the water to eliminate pooling water.
2. Clean and remove all grease build-up under the grill. Grease build-up becomes difficult over time to remove. Clean the area regularly to prevent the formation of grease. Grease build-up not only attracts pests, but can cause fire,

Observation: 1. Under the grill and under shelving holding rice beside walk-in cooler still observed with long standing debris and grease build-up on floors.
2. Dust build-up still observed above vents in kitchen area.
Corrective Action(s): Clean all areas and remove all grease, dust, and debris to maintain the premises in sanitary condition and prevent the attraction of pests. Date to be completed: by next inspection

Observations During Inspection:
- Dead cockroaches removed from under front counter in front area, in bathroom, and beside hot water tank
- Pest control report emailed to inspector
- Operator purchased new nozzles for hard liquor bottles to prevent pest entry
- No food observed stored directly on the floor
- Cut vegetables timed stamped for 2 hours
- Back door closed
