Observation: dead cockroaches noted behind the soda pop shelf in the front of house, in the knife block in the back of house, and under the front of house handsink. operator has the facility serviced every 2 weeks, pest control reports noted that the issue is getting smaller however leaks in the facility must be repaired to discourage pests
Corrective Action(s): clean the above noted areas and follow all recommendations of the pest control service

Observation: Utensil basket under the grill is greasy, food debris build up noted on storage shelves behind the prep cooler, food debris and mold noted under the central handsink, dishwasher filter (below unit) has moldy food debris inside, and grill area catch tray requires cleaning
Corrective Action(s): Deep clean the facility on a more frequent basis. operator must clean the above noted areas. due: 2 days
